This is not a module, but rather, changes to the core files.
This Modification was originally developed as a “Special Instructions” text area for a restaurant taking online orders.

While it can be used as a simple Textarea for the user to enter special instructions or requests, it is a powerful Modification that can be used for a much more complex product customization (especially for those needing multiple attributes and form elements other than the default drop downs).
A good example can be seen at http://lensesrx.com. If you click “BUY NOW!” on a pair of glasses, you will see a popup for customizing the glasses (lens type, prescription, extras, etc..), which, at the end, gets added to the cart along with the product.
